2px;'>8月2日，我们准备使用PRP疗法处理姚林芳再次延迟愈合的创口。PRP疗法的中文名称叫富血小板血浆疗法，大致流程是抽取少量自体血，一般30ml~50ml足矣，将其中富含生长因子及血小板的部分提取出来形成凝胶，可以用于创面修复，也可用于关节软骨损伤的修复等。我们手足外科和烧伤科都开展创面修复技术，烧伤科更精于浅表创面或大面积创面的修复，我们更专注于涉及深部组织感染或缺损创面的修复，两个科室有交集，但各有侧重，PRP技术烧伤科经验更为丰富，因此今日由烧伤科同道主持PRP操作。PRP凝胶需要覆盖在创面长达5~7天以上，在此期间发生感染将导致治疗的失败，而创口临近会阴，更容易被污染，经过讨论，决定预防性使用抗生素降低感染几率。</span></section><section 2px;'>根据姚林芳细菌培养及药物敏感试验的结果，抗生素可供选择的品类很少，青霉素类是有效品种之一，姚林芳住院期间曾经使用过青霉素，第一次用药期间无任何异常，第二次青霉素皮试时出现手指尖瘙痒的感觉，这种情况被称为青霉素皮试阳性，并非青霉素过敏，因此我们准备在密切观察的基础上再次进行青霉素皮试。</span></section><section style='margin-right: style='letter-spacing: 2px;'>随着皮试药液注入皮下，姚林芳在数秒后迅速出现烦躁、声音嘶哑，皮肤瘙痒的感觉从四肢迅速向躯干扩散，我的第一反应就是很可能发生过敏性休克了！立即呼叫值班医生护士准备抢救，呼叫当值麻醉师急会诊，做气管切开的准备，姚林芳声音嘶哑迅速加重，并出现神志模糊及恶心呕吐的症状，抢救车被迅速推到床边，按口头医嘱立即给予盐酸肾上腺素1mg肌注，地塞米松10mg静推，用药后不到一分钟，姚林芳整个人已经神志转为清晰，声音嘶哑明显好转，全身瘙痒也逐渐缓解。</span></section><section 2px;'>从皮试药液注入，到休克发生，到药物注入后病情缓解，全程可能只有两三分钟分钟左右。姚林芳是为了抢救而注射的肾上腺素，而我自己体内，也在那一瞬间大量分泌肾上腺素，导致我心率骤增，脑部血供增加，短时间内思维更敏捷，行动更迅速。事后回想还是有点后怕的，在电光火石的刹那，如果没有意识到正在发生过敏性休克，后果将不堪设想。为什么我对姚林芳声音嘶哑如此关注，因为过敏性休克往往迅速发生喉头水肿，喉头水肿的早期表现就是声音嘶哑，而姚林芳声音嘶哑快速加重证实了我的猜想，如果喉头水肿继续进展，很快就会出现窒息，到了那个阶段，唯一的办法就是立即气管切开恢复呼吸，这也就是我第一时间呼叫麻醉师准备气管切开的原因。如果喉头水肿没有及时处理，就会因为脑部缺氧导致最坏的结果，这将是所有人都无法承受的后果。</span></section><section 2px;'>刚工作时听高年资医生护士聊过，听说有的人只要附近有打开的青霉素安瓶，闻到一点青霉素的味道就能休克，当时听闻觉得好夸张，后来在漫长的工作生涯中，我经过各种层次的学习、培训，最终我虽然没有真正碰到并处理过敏性休克的患者，却在脑海中对过敏性休克的发生发展、诊断及治疗有了一个大致清晰的轮廓，因此我才能在那一瞬间做出判断及处理，治疗的结果也证实我的判断是正确的。</span></section><section 2px;'>比如海姆立克急救法，在座的各位都熟练掌握了吗？要知道，一旦呼吸道进入异物发生窒息，死神就按下了倒计时按钮！因为窒息发生之后的1~6分钟随时会发生心跳骤停，超过6分钟，大脑就会出现不可逆死亡，您如果只知道赶紧把患者送到医院，至少要花十几分钟甚至几十分钟吧，而等送到医院，一切为时已晚！唯一的办法就是现场立即使用海姆立克急救法当场排出异物解除窒息。每每听闻有些小孩吃东西窒息后，被家长万分火急送到医院，却无力回天，就觉得万分心痛。</span></section><section 2px;'>我们让时光回转，故事重来一遍：在偏远山区有一名年轻人因为流感继发病毒性心肌炎突然心跳骤停倒在马路边，他身边的十几个热心亲戚朋友每个人都很关心他，都很着急，他们拨打了120。巧的是，同伴中正好有一位先生曾经在单位接受过心肺复苏的培训，他不是医生，碰到这种情况非常紧张，但还是勇敢的站了出来，他紧张的回忆每一个步骤，他依稀记得讲师说第一步应该判断是不是真的心跳骤停了，于是他拍打患者的肩膀，大声呼喊病人，病人毫无反应！接下来他用两根手指摸到患者颈部正中的气管，向旁边侧滑按下去，这是颈动脉的位置，几秒之后，他没有感觉到任何搏动！此时可以确定小伙子心跳骤停了！他立即在小伙子身旁双膝跪地，两手手指交叉、掌根重叠放在小伙子胸前乳头之间，使用整个上半身的重量往下压，按照讲师所要求的，按压频率每分钟100次以上，每次下压5cm以上，第一次真正持续做这个动作，他突然发现这个动作运动量是如此之大，很快我们的施救者便气喘吁吁，但是他坚持按照培训的步骤继续心脏按压。从他按下去的那一瞬间，小伙子的心脏被强大的外力挤压，心脏内的血液喷涌而出，全身血管内再次出现血流！我们的施救者虽然慌乱，但是理智告诉他应该继续坚持，按压30次以后，他进行两次人工呼吸，如此循环操作持续了两分多钟，旁观者突然发现小伙子苍白的脸色转为红润，再次触摸颈动脉有了搏动！在他们的呵护下，救护车呼啸而至，医生接手了抢救工作，小伙子经过精心治疗，最终痊愈出院。</span></section><section
